Updating Vendor Information (W9 Form)


Why update my Vendor Information?

An updated W-9 is needed for:

  1. Change of Address
  2. Tax ID Update, and/or
  3. DBA Change

Start by downloading the W-9 form.

Complete the downloaded W-9 with the following information:
  1. Your current name or business name
  2. DBA
  3. Address
  4. Tax ID, and
  5. Tax Status
  6. Your signature and Date

Submitting W-9 changes is easy

  1. Simply print, complete and sign the W-9 form.
  2. Scan and email the completed/signed form to SupplierSetup@traviscountytx.gov.
  3. Or you may mail it to:

    ATTN: Disbursements Auditing
