Updating Vendor Information (W9 Form)


Why update my Vendor Information?

An updated W-9 is needed for:

  • Change of Address
  • Tax ID Update, and/or
  • DBA Change


Start by downloading the W-9 form.    Click Here

Complete the downloaded W-9 with the following information:
  • Your current name or business name
  • DBA
  • Address
  • Tax ID, and
  • Tax Status
Submitting W-9 changes is easy

  • Simply print, complete and sign the W-9 form.
  • Scan and email the completed/signed form to AP@traviscountytx.gov.
  • Or you may mail it to:

    ATTN: Disbursements Auditing
